Domestic Violence and Endangered

We will not discriminate against a covered person solely because they are or have been a victim of domestic violence.  We will not refuse to issue or renew, deny or cancel coverage; demand or require a greater premium or payment; designate domestic violence as a condition for which coverage will be denied or reduced; or fix any lower rate or discriminate in the fees or commissions of producers for writing or renewing such a policy.

If any person covered by our group insurance policy delivers to our home office, a valid order of protection against the policyholder or other person covered by our policy, then we will not disclose to the policyholder or other person the address or telephone number of the insured or of any person or entity providing covered services to the insured for the duration of the order.  If the covered person is a child, these rights may be asserted by the child.

We will accommodate a reasonable request made by a covered person to receive communications of claim related information by alternative means or at alternative locations if the person clearly states that disclosure of all or part of the information could endanger the person. If a child is covered by us, then the child’s parent or guardian may make such a request.  This request may be made by telephone, fax or in writing to us.

Except with the express consent of the person making the request, we will not disclose to the policyholder or another insured covered under the policy: (1) the address, telephone number, or any other personally identifying information of the person who made the request or child for whose benefit a request was made; (2) the nature of the health care services provided; or (3) the name or address of the provider of the covered services.

The covered person may revoke this request by contacting us by telephone. As part of the verification process, the covered person who originally made the request will be asked to provide us with the alternate address that was initially disclosed to us, or a password assigned to the requestor.
